Are you new to Burleigh Dental Studio?
Welcome! Before we see you, we’ll need some personal information and details of your dental history in order to provide you with the best level of care.
Why is it necessary for us to obtain this information?
Knowledge of your medical history allows us to recognise any existing conditions or concerns prior to your appointment.
This enables us to identify those at increased risk of any health problems, thus taking the important steps to help reduce that risk.
By answering all questions thoroughly and accurately to the best of your knowledge, you will be helping our team ensure the best possible care and bespoke treatment plans for you.
Don’t worry, your personal records are kept confidential and are held in strict confidence. They will not be disclosed to any third party (subject to applicable laws).
PAYMENT OPTIONS
Premium general, cosmetic and implant dentistry at an affordable price? Yes you can have it all. We pride ourselves on superior service and treatment outcomes without the inflated cost, by maintaining our dental fees within industry standards.
It’s important to us that your time at our studio is an accommodating and pleasant experience from start to finish. For your convenience, we accept cash and EFTPOS Mastercard or Visa payment methods. We are a HICAPS provider with the ability to process health fund rebates on the spot for the vast majority of Australian health insurance funds.
Our priority is always your comfort, wellbeing and dental health. You are informed and involved in the development of your treatment plan in order to ensure your comfort with the direction, the level of care, and the value for money.

Burleigh Dental Studio is proud to be a Member’s First Preferred Provider for CBHS and Westfund. We accept all major Australian health insurance funds and can process on-the-spot health fund rebates as a HICAPS provider.
